I wonder if that was sent to store owners or regular consumers. I've never seen that specific form before, but I'm pretty sure Tecmo or Capcom also had a direct order slip packed in with one of their games. Anyways, I'll see if I can find the insert I'm thinking of and post a picture later.

I don't recall Capcom having a direct order slip like that in any of their games. If you ordered directly from them through the mail maybe they would include it.

Yeah, I looked through about ten games and couldn't find what I was thinking of. Perhaps someone with a better memory will be able to recall. Or I'm just crazy.

I got this when I was a kid, I remember it came in the mail with a book. My dad signed up for every game mailing list, I remember also getting stuff from Squaresoft . We ended up buying both Ninja Gaiden games through this since I liked the book, which I definitely still have somewhere in my attic. I don't think this was the only one we got, maybe one more because we also got a free Blaster Master book. I'm gonna ask my dad and my brother and see if they remember anything more.
