Cryptic Continues? Games that do not have an obvious continue option.

The "cryptic continue" is likely the result of late feature introduction. By the time they realized continuation would be beneficial, it was too late to redesign the menu screen. So, they wedged in the feature via a button combo.

A similar thing exists in NES Tetris. To start on levels 10--19, you have to select 0--9 and then press A+Start rather than Start. Nothing on the screen indicates this. Though, the manual mentions it.

For some reason, I thought that a lot of older NES games had that feature to continue.  However, Rad Racer and SMB are the only ones I know of for sure that had it.
There are some unlicensed games that have it too. I believe Castle of Deceit and Moon Ranger are two of them. Challenge of the Dragon is another -- it even tells you to "PRESS A + START TO CONTINUE" onscreen after you get a GAME OVER.

EDIT: GameFAQs says you hold A+Start to continue in Casino Kid, but I've never played it so I don't know if that's true.

And, Seicross lets you continue with A+Start.

Continue codes were also put there for play testing purposes. That's why the Konami code was invented.

Yeah, I think test-codes that don't get removed (for fear of fucking up already tested code) is a way more likely explanation for pretty much all continue and level-select codes.

Wow, I never knew about Gradius, Double Dragon 2 or Milon's Secret Castle, and those are all games that I have played a lot.

In Adventure Island, you have to get the secret egg at the end of the first stage. Then you will be able to continue by holding right and pressing start on the title screen.
