I noticed in my Bases Loaded repro, FCEUX, and Jnes, the game crashes when you complete stage 1-5 as anything other than fire mario. I've tried different sources for both the rom and the patch and they produce the same error. 
Edit: Now it seems to crash at random. No difference between with or without save states, emulator or real hardware. 

Edit #2: Finally found a ROM and patch combination that could not produce a crash in any of the emulators I tested. if you do the konami code before the title screen finishes scrolling, level select is enabled. press B to cycle through the world that you would like to start on. you will see a number appear near the top of the screen. or press A+start to play the game in hard mode
